"Hello everyone!
Let me introduce myself. My name is Alexander, I live in Moscow, Russia. I'm 24 years old. My story lasts for 2.5 years. To be honest, I came here by advice (stevo, thank you again) from another forum where I was discussing surgery as a last resort for my case. Hope this forum could help me to find more options.
Here's a history of my case.
I'm almost sure cause of my condition was overdoing sex 2.5 yrs ago. Till then I hadn't regular girlfriend for a year or so - masturbated maybe 3-4 times a week, sometimes 2 times in succession. And few days after I met my girlfriend, she was living at my place for 3 or 4 days. And we had sex sometimes 2 or 3 times in a row. And one more thing - usually it was hard for me to reach a climax, I was used to - so sex could last for about a 30 minutes. 3 days later in the morning I went to the bathroom and felt burning pain when was peeing. When I finished there was burning in urethra and feeling that I still have urine left in my bladder plus discomfort and irresistible urge to urinate despite of empty bladder. This was reaally horrible feeling. Few minutes later pain left me, but uncomfortable itchy sensation in the back of my urethra and in perineal zone became persistent.
Worst of all - 2 or 3 days later symptoms returned and new added. That's the symptoms I have now. I have burning pain in perineum especially when sitting on a hard surface or driving, wearing tight pants. Burning in the back of urethra - sometimes visiting toilet I feel like I peeing out barbed wire, feeling of "last drop" staying in urethra after visiting toilet. Sharp shooting pain (like electric discharge) through urethra to the tip of my penis. Sometimes heavy discomfort in perineum like my bladder is going to blow, but when I'm near bowl there are just few drops. Ejaculation sometimes painful, and is happening much faster than before. Burning after sex is sometimes unbearable. . . So now I often try to avoid sex and this makes me really depressed. Lifting heavy things makes my symptoms worse, lying on a back gives me relief - I'm going to the bathroom in night very seldom because of it.
Now about my tests and treatments I have tried. You'll see that I'm considering surgery for a reason. . .
I have high WBC count (30 in moderate) and enterococcus faecalis (10koe4) in EPS, DRE is painful. No STD. TRUS shows area of calcification in the periurethral zone in the right lobe of prostate. Nothing special on CT and MRT. Myography and uroflowmetry are ok. Flow is good, no intraprostatic reflux. And few weeks ago I had a prostate biopsy done - calcified zone samples has signs of inflammation and fibrosis, also presence of bacteria. PSA is 0.4.
I've tried antibiotics (all quinolones and much more), alfa-blockers, heat therapy, magnetic therapy, SANOS protocol, prostate massage, broccoli thing, changings in diet - no caffeine, alcohol etc, cauterization of colliculis seminalis with AgN (that's hellish thing, only some old soviet school docs uses this), biofeedback, Russian analog of saw palmetto, pollen bee, hot baths, detralex and troxevasin (I have dilated (varicose) periprostatic veins - does anyone have it also? seems important to me), vezikar ( (SOLYFENACINUM - anti-hyperactive bladder med), diclofenac, other anti-inflammatory meds, enzymes, allopurinol, also katadalon, grandaxin and sirdalud - this is a muscle relaxant, painkiller and neuroleptic from a neurology specialist. . . Even hirudotherapy and homeopathy. Not sure this is a complete list. Everything failed or at had very short-time effect.
I found uro who was giving me intraprostatic shots using same cocktail of abx and steroids as Bahn - I found recipe on Bahn's page and gave doc a link. He gave me 3 shots with 10 days interval - after last injection my symptoms reduced maybe by 50%, but came back with revenge within a month. He is expensive, I can't allow doing injections for the rest of my life every 2-3 months, so I quit.
Now I'm going to pool and swim 2-3 times a week, exercise at tai-chi class, doing Kegel exercises - all for about 4 months. Still no help and I'm loosing heart because of it. There are no PT qualified in trigger point release in Russia, and I don't have a possibility to go to Stanford clinic, but I'm going to order a copy of "A Headache in the Pelvis" tomorrow. I decided to put off surgery for a few months at least and look for other possibilities. But if nothing helps. . .
So I'll be glad to hear any suggestions and advices. Anything."

Z doktorem można komunikować się po angielsku i rosyjsku oczywiście. Super człowiek doskonały lekarz posiadający wielką wiedzę i przede wszystkim kompetentny.
